In fact, there are many people around us who go to the hospital because of knee pain, shoulder pain and heel pain and ask the doctor to give a closed injection. Is blocking needle analgesic? What’s the secret in it? Here, pharmacist Yin Jiayu from the Pharmaceutical Department of Guangzhou first people’s hospital shows you the “magic” of closed needle.

First there was pain, then there was the closed needle,

Pain is the “fifth vital sign” after respiration, pulse, blood pressure and body temperature. When the body is injured, we feel pain.

Among them, orthopedic pain is one of the common clinical symptoms, such as fracture, muscle strain, ligament injury, pain can cause our psychological and physiological discomfort, can lead to the decline of quality of life and psychological negative emotions. Therefore, it is very important to use appropriate analgesic methods. The methods of clinical orthopedic pain treatment include drug therapy and non drug therapy (physical therapy, relaxation therapy, etc.). And blocking therapy is one of the commonly used ways of drug treatment.

Yin Jiayu said that for orthopedic pain, blocking therapy is to inject a certain concentration or dose of drugs into the lesion areas, such as joints and fascia. Closed treatment is an effective treatment for acute and chronic pain in orthopedics department. It has the advantages of simple operation, significant effect and low cost.

Yin Jiayu said that the blocking needle is mostly made of prednisolone and other glucocorticoid drugs mixed with local anesthetics and neurotrophic drugs. These three drugs are very helpful to relieve local pain.

(1) Glucocorticoid

Glucocorticoid is a kind of steroid hormone secreted by adrenal cortex, which has many pharmacological effects such as anti-inflammatory, anti allergic and immune suppression. The effect of local block therapy with glucocorticoid is good. Such drugs are dexamethasone, betamethasone, triamcinolone acetonide and other drugs. However, it should be noted that long-term large-scale use of hormone can cause adverse reactions, such as inducing or aggravating infection, leading to muscle atrophy, osteoporosis and so on.

(2) Local anesthetics

Local anesthetics are a kind of drugs that can reversibly block the occurrence and transmission of sensory nerve impulse at the site of medication, and reversibly cause the disappearance of local tissue pain.

These local anesthetics can cut off the conduction of pain nerves, relieve and inhibit pain, improve circulation, dilate blood vessels, improve blood flow, and relieve muscle spasm. Such drugs include lidocaine, procaine, ropivacaine, etc.

(3) Vitamin B12 and Mecobalamin

The traditional blocking therapy is local anesthetics combined with glucocorticoids. Although the curative effect of treatment is remarkable, it is still insufficient for the treatment of orthopedic pain. General clinicians will add “neurotrophins”, vitamin B12 or Mecobalamin.

Vitamin B12 is involved in the synthesis and maintenance of myelin sheath around nerve cells and the synthesis of red blood cells. It can increase nutrition and promote the recovery of nerve, muscle, tendon and other soft tissues. Mecobalamin is also a member of vitamin B12 family, which can promote the repair of damaged nerve.

The side effect is not small when the blocking needle is used more.

When injecting the blocking needle, the diagnosis should be clear, and the curative effect is not good for the non indication orthopedic diseases. For example, it can not be used for pain caused by other diseases, such as cancer pain, etc., which is easy to delay the illness.

Secondly, the interval of closed treatment should be 7-10 days, and continuous injection should not exceed 3-4 times. It is emphasized here that we should not rely on treatment and should not use it repeatedly in a short time, otherwise it may cause osteoporosis and even aggravate the disease.

After repeated application of large dose, it can cause centripetal obesity, body shape change, hairy skin, slow down wound healing, induce hypertension and other side effects.

Need to remind is, closed needle need strict aseptic operation, to the regular medical unit to do closed treatment.

Finally, Yin Jiayu said that drugs are not omnipotent. Without proper rest, any drug treatment is empty talk. Therefore, if there is pain after the injury, this is a warning. We need to rest as soon as possible, instead of going on the court with a disease after a closed needle.
