Interventional therapy? You know what? Maybe in the hospital, you never pay attention to this department.

From September 4 to 8, sponsored by Guangdong Medical Education Association, the 8th China pediatric interventional radiology academic conference was held in Guangzhou, which was organized by pediatric interventional Committee of cancer intervention branch of China Anti Cancer Association, women and children interventional group of interventional Committee of China radiology branch and Department of intervention and hemangioma of Guangzhou Women and children medical center.

Professor Zhang Jing, chairman of the conference and director of intervention and hemangioma Department of Guangzhou Women’s and children’s Medical Center, said that interventional therapy is a rapidly developing discipline. Minimally invasive treatment guided by various channels and local images in the human body has a closer relationship with obstetrics and Gynecology and pediatrics, which has brought significant changes to the diagnosis and treatment of many diseases. However, due to the different qualifications and equipment of medical institutions at all levels, there are many problems in the actual operation of interventional therapy, especially the standardization of treatment needs to be improved.

Interventional therapy of “internal and external repair”

Interventional therapy is a new subject which combines imaging diagnosis and clinical treatment. It is under the guidance and monitoring of digital subtraction angiography, CT, ultrasound and magnetic resonance imaging equipment, using puncture needle, catheter and other interventional equipment, through the human body’s natural channels or small wounds to introduce specific equipment into the human body lesions for minimally invasive treatment. At present, it has become one of the three main clinical disciplines in parallel with traditional internal medicine and surgery.

“It’s a treatment between surgery and internal medicine.” Professor Zhang Jing said, including intravascular intervention and non vascular interventional treatment. Because interventional therapy has many advantages such as simple, safe, effective, minimally invasive and less complications, it has been applied in many diseases of Pediatrics and Gynecology, such as children’s hemangioma, vascular malformation, retinoblastoma and other malignant tumors, as well as the prevention and treatment of gynecological tubal infertility, uterine fibroids, cervical cancer, postpartum hemorrhage, etc It’s one of the treatments.

Professor Zhang Jing said that as long as it is a top three hospital, there are basically intervention departments. For example, Guangzhou Women’s and children’s medical center also set up specialized outpatient department of vascular intervention, and adopted multi-disciplinary consultation mode to serve patients.

Professor Zhang Jing, chairman of the conference and director of intervention and hemangioma Department of Guangzhou women and children’s Medical Center,

These diseases are most suitable for interventional therapy,

Nowadays, interventional therapy is widely used, liver, cardiovascular and other disciplines are developing, but the relationship with obstetrics and Gynecology and pediatrics is more and more close. “Children with hemangioma, vascular malformation, congenital diseases. The vast majority of parents take their children to many departments, including dermatology, plastic surgery, pediatric surgery, etc Zhang Jing said, “this disease is treated in the Department of hemangioma, the cure rate is as high as 99%, but the result is because parents don’t know that they can’t get standard treatment.”

Which diseases are suitable for interventional therapy? What are the advantages? Professor Zhang Jing gives the following suggestions:

Prevent postpartum hemorrhage and avoid hysterectomy

Clinically, postpartum hemorrhage is the primary cause of maternal death in China. Postpartum hemorrhage is a serious complication of childbirth. In the past, when conservative treatment failed, we had to save the lives of patients at the cost of hysterectomy. After hysterectomy, although the patient’s life is saved, it will cause a series of physiological and psychological changes. In some critical cases, patients do not even have the opportunity to remove the uterus.

The appearance of interventional therapy has completely changed this situation. Qualified hospitals have become the preferred treatment. Director Zhang Jing further pointed out that gynecologists can prevent the occurrence of postpartum hemorrhage through the evaluation of pregnant women, such as placenta accreta, placenta previa, scar pregnancy and other high-risk groups of postpartum hemorrhage through the intervention method of prenatal uterine artery balloon preset. At the same time, cervical cancer, uterine fibroids and other common gynecological diseases can choose interventional treatment.

The cure rate of hemangioma and vascular malformation in children can reach 99%

The incidence rate of vascular malformation in children with hemangioma is about 5%-10%, of which 50%-60% is all in the maxillofacial region of infants and young children, and the most common part is the face and neck. In addition, many other parts of the body, such as the tongue and brain, may also grow.

Professor Zhang Jing emphasized that the incidence rate of vascular malformations of hemangioma is high, so long as the treatment is right, the cure rate is 99%. For example, in hemangioma, the blood vessels are like tree roots, and the branches and branches are intertwined, which is difficult to remove and is not suitable for surgical treatment. Interventional doctors through the method of vascular intervention, drugs can reach each treatment site along the blood vessels, more thorough treatment. In addition, the following two misunderstandings should be kept in mind in the process of seeing a doctor: first, don’t think that the initial hemangioma is not obvious or the tumor is small and you don’t pay attention to it. You should consult the doctor’s opinion for scientific treatment at the first time; second, don’t rush to the doctor and choose the appropriate treatment method to avoid over treatment.

Interventional treatment of retinoblastoma can protect the eyeball

Interventional therapy has become the most mainstream treatment for common malignant tumors in children, such as retinoblastoma, that is, catheterization via femoral artery, selective insertion of catheter into ophthalmic artery, and ophthalmic artery infusion chemotherapy. In this way, the drug concentration for tumor tissue is high, while the drug concentration for normal tissue is low, so as to reduce the systemic toxicity.

In addition, patients’ eyeballs can be preserved by interventional therapy. Professor Zhang Jing pointed out that in the treatment of retinoblastoma, the eye extraction rate in China is significantly higher than that in foreign countries. In developed countries in Europe and America, the eye extraction rate of patients is only 6% – 12%.
